What should I bring to my first appointment?
Bring a valid photo ID and your insurance card if you have one. If you completed your new patient forms online, you're all set. If not, just arrive about 15 minutes early so you can fill them out before your appointment.
Can I fill out my forms before I arrive?
Yes. We recommend completing your new patient forms online before your visit. It saves time at check-in and lets Dr. Jensen review your health history ahead of your appointment so she's prepared before you sit down.
How do I schedule or reschedule an appointment?
You can book online anytime through our scheduling page, call us at (816) 605-0034 during office hours, or text us at 816-495-4073 after hours. We'll get back to you as soon as possible.
Do you accept my insurance?
We accept most major PPO dental insurance plans including Aetna, Delta Dental, Blue Cross Blue Shield, MetLife, Cigna, Guardian, UnitedHealthcare, and more. Our team will verify your benefits before your visit so you know what's covered.
What if I don't have insurance?
No problem! We offer an in-house membership plan starting at $360 per year for individuals that includes exams, cleanings, fluoride treatments, X-rays, and 25% off all procedures. Family and couples plans are also available.
What are your office hours?
We're open Monday, Tuesday, and Thursday from 8 to 5, and Wednesday from 9 to 6. We're closed Friday through Sunday. For after-hours concerns, text us at 816-495-4073 and we'll get back to you.
